In 2016, researchers at <a href=\"https:\/\/www.hopkinsmedicine.org\/news\/media\/releases\/study_suggests_medical_errors_now_third_leading_cause_of_death_in_the_us\" target=\"_blank\" rel=\"noopener\">Johns Hopkins<\/a> estimated that in the US as many as 250,000 people die each year due to medical negligence. If this statistic is accurate, it makes medical errors the third-leading cause of death in the US.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-840255d elementor-widget elementor-widget-text-editor\" data-id=\"840255d\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>Critics of the Johns Hopkins study report argue that the analysis was flawed as there is no standardised method for collecting national statistics for incidence rates for deaths directly caused by errors.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-2820697 elementor-widget elementor-widget-text-editor\" data-id=\"2820697\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>One of the challenges is that there is no universally accepted definition of \u201cwhat constitutes a medical error\u201d. In other words, it\u00b4s a failure in medical planning or execution that contributes to an unintended preventable result. Examples include<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/29763131\/\" target=\"_blank\" rel=\"noopener\">*<\/a>:<br><br>\n<ul>\n \t<li>Failing to complete the intended plan of action or implementing the wrong plan<\/li>\n \t<li>An unintended act or one that fails to achieve the intended outcome<\/li>\n \t<li>Deviating from agreed processes, which may or may not result in harm<\/li>\n \t<li>When planning or performing a procedure, doing anything that causes an unintended consequence<\/li>\n<\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-73ad29c elementor-widget elementor-widget-text-editor\" data-id=\"73ad29c\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>It is important to distinguish the type of medical error being discussed, as not all medical errors are human errors. This is done through root cause analysis and comprehensive reviews of the collected data. With insights from previous and repeated errors, new guidelines and processes can be introduced to reduce medical errors and medical negligence.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-fa0c5fa elementor-widget elementor-widget-text-editor\" data-id=\"fa0c5fa\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>There are challenges with self-reporting systems however, as they rely on personal accountability and several other factors, as outlined by the <a href=\"https:\/\/www.ihi.org\/resources\/Pages\/Measures\/NumberofSelfReportedMedicationErrors.aspx\" target=\"_blank\" rel=\"noopener\">Institute for Healthcare Improvement<\/a>:<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-c878324 e-flex e-con-boxed e-con e-parent\" data-id=\"c878324\" data-element_type=\"container\" data-e-type=\"container\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-10adde0 elementor-blockquote--skin-border elementor-widget elementor-widget-blockquote\" data-id=\"10adde0\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"blockquote.default\">\n\t\t\t\t\t\t\t<blockquote class=\"elementor-blockquote\">\n\t\t\t<p class=\"elementor-blockquote__content\">\n\t\t\t\t\u201c <i>First, staff must recognize when errors occur. experience of self-reporting medical errors, M3 Global Research conducted an internal survey with 3,700 M3 panel members across 14 countries in Europe and USA, between July and September practices. e-child\" data-id=\"98001df\" data-element_type=\"container\" data-e-type=\"container\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-3e0be58 elementor-widget elementor-widget-text-editor\" data-id=\"3e0be58\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<ul>\n \t<li>60% of respondents feel comfortable admitting to, and reporting a minor medical mistake after they have found a solution, and 50% would feel comfortable reporting the error even if they had not been able to resolve it.<\/li>\n \t<li>In the case of severe medical mistakes that they were able to resolve, respondents were divided in terms of whether they felt comfortable reporting them (49%), or uncomfortable, but reporting the error regardless (45%).<\/li>\n \t<li>90% would report severe medical mistakes even if they weren\u2019t able to resolve it.<\/li>\n \t<li>When it comes to reporting medical mistakes made by co-workers, 35% would feel uncomfortable reporting minor or severe medical errors made by their colleagues, but would do so to improve the
